Existing Damage Form

Does anyone use an existing problems form to list damage to property before starting jobs. I was trying to develop one and if anyone has already created on that I could modify would be greatly appreciated.

We note existing damage on the proposal. It helps alot. If you note there is existing overspray, well, they can't exactly blame you for it can they....
