projectile nozzle

So I am washing a wood table today at a customers house. Changed nozzles and when I pulled the trigger the nozzle shot out of the lance hit the table and went flying into the yard somewhere ( did not find it) it also took the o ring with it. So I put a new o ring in and new nozzle and kept on working. am wondering if I just did not lock it in tightly or maybe the fitting is going bad? Anyone ever had this happen to them?
 
I had a employee break a rear window in a hummer on the car lot about 2 years ago. $800 window. :(. It used to happen now and when the would toss the wand down to move the hose. The quick connect would hit just right and cause the tip to take flight the next time the trigger was pulled.
 
99.9% you probably didnt place the tip in all the way. (that sounds so wrong). Anyway I have been guilty of this as have my guys. Always make sure its snapped in correctly. We ALL also make a habit of aiming it downward and away from anything such as vehicles, windows etc... before we start washing. I've lost a few tips that have ended up in neighbors yards before. It will almost always take the o-ring with it too.
 
Larry is right, the nozzle was not in all the way.

Have everyone make sure that the locking sleeve is all the way down and flush with the other metal part.

I have had helpers connect the ball valve to the gun/wand or the surface cleaner and it pops apart and they blow out the o-ring, when the o-ring is gone then I know that they did not have it connected all the way.

It does not happen that much anymore as the guys hate getting soaked and wet.

Had that happen to me once, ball valve whipped and nailed me in the gut. Bout took the wind out of me.

WTH is headlined......man I need to sleep more often!

I meant to say hard lined...:smash: (there's no stink'n couplers) at the end of my wands.:indecisive:
 
First time I shot the nozzle it went and hit a steel door. The people inside came out thinking someone had fired a gun. Was hilarious in hindsight. Replaced the end QC cause it was a bit worn. I wasn't able to lock it in properly.
