need input on new website feature

Looks like a more attractive "contact us" form. I think "contact us" started as a place to get contact info...now it's abused as a con for us to get their contact info...kind of sketchy, I rarely fill them out personally. If your number or email isn't on there, I rarely fill out the form...even if my site has the same con on it.
Your "submit a ticket" is a better description and much more inviting than the "contact us" or "tell us everything about you form including your mother's maiden name and your spouse's blood type".

That said, I like it!
 
If you need something like that it is good. You have your phone number so people can call you. The ones who don't feel like talking will like it and use it. People who you work with regularly might just use your email, unless you direct them to that. I am guessing you really actually have staff and it will be sorted out to the right person automatically. People who are busy might use it to save time and avoid small talk.

This is for people to use if we are servicing them weekly say one of our PM's that has signed up for lawn maintenance. If one of the techs misses a spot or something something of that nature she can have this page bookmarked and bam! Instant customer service. The bugs and support goes to everyone as well as emergency. The others get sorted to me for sales or to my office manager.

Especially if they use a smart phone.
